What Is Underwriting?

The one-minute explanation of underwriting — no jargon.

Underwriting: The insurer's risk-evaluation process — mostly banned for ACA health plans (which can't use it to deny or price you), but still used for life, disability, and supplemental products.

In Plain English

Underwriting is how insurers evaluate risk before selling a policy: health questions, medical exams, prescription history checks. For ACA Marketplace health plans, medical underwriting is banned — they can't ask, can't deny, can't price on health. But underwriting is alive and well for life insurance, disability, long-term care, and many supplemental products.

A Real Example

You apply for a Marketplace health plan: zero health questions, same price as everyone your age. Then you apply for a standalone critical-illness supplement: full health questionnaire, prescription history check, and a possible decline or rated premium based on your answers. Same person, two products, two completely different underwriting worlds.

Why It Matters When You Shop

Underwriting determines when to buy: supplemental and life products are cheapest and easiest to get when you're young and healthy — buy them before you need them. Waiting until a diagnosis means higher prices or unavailability for everything except ACA health plans.

Watch Out For

Answer underwriting questions honestly and completely — misrepresentation can void a policy exactly when you need it most. And if a "health plan" asks detailed medical questions before quoting, it's not an ACA plan; it's underwritten, and you should know what you're actually buying.

Common Questions

Will applying for supplemental insurance affect my health plan? No — they're separate applications with separate insurers (usually). A decline on a supplement doesn't touch your ACA coverage.

How far back do underwriting questions go? Typically 5–10 years for major conditions, less for minor ones — it varies by product and carrier. Answer the questions as written; don't volunteer beyond them, but never omit what's asked.

Can I be declined for a supplement but approved for the health plan? Absolutely — they're different risk pools with different rules. The health plan can't decline you; the supplement can.

Put It to Work

The strategic move with underwritten products is buy young, buy healthy: lock in supplemental and life coverage in your 20s–30s when underwriting is a formality and premiums are lowest. Every year you wait, the questions get harder and the prices get higher. And keep copies of every application you sign — if a claim is ever questioned, your accurate, complete application is your shield.

How I Explain It to Clients

"Buy the underwritten stuff while you're young and boring," I advise. "Your ACA health plan will take you anytime, no questions asked — but life insurance, disability, and supplements get harder and pricier with every birthday and every diagnosis." The calendar is the underwriter's best friend and the procrastinator's worst enemy.
