What Is External Review?

The one-minute explanation of external review — no jargon.

External Review: An independent review of a denied claim by medical experts outside your insurance company — their decision is binding on the plan.

In Plain English

When your plan's internal appeal upholds a denial, you can take it to external review: independent reviewers with no ties to your insurer examine the medical facts and decide who's right. If they side with you, your plan must cover the service. It's free to you, and it's the great equalizer — your doctor's judgment weighed against the plan's, by neutral judges.

A Real Example

Your plan denies ongoing physical therapy after 12 sessions, calling more "not medically necessary." Your therapist documents measurable progress and the risk of regression without continued care. External reviewers — licensed clinicians in the relevant specialty — review the records and overturn the denial. The plan authorizes 12 more sessions.

Why It Matters When You Shop

External review rights come standard with every Marketplace plan under federal law. It's one of the strongest consumer protections in the system, and most people have never heard of it — which is exactly why insurers would prefer you didn't know.

Watch Out For

You generally must complete the internal appeal first (except in urgent cases, which can go straight to expedited external review). The request window is typically four months from the final internal denial. Your state's department of insurance can help you file — ask me and I'll point you to yours.

Common Questions

Who are these reviewers? Independent medical professionals in the relevant specialty, assigned through accredited organizations — not employees of your insurer, not paid by them for the outcome.

Does it cost anything? No — external review is free to you. The plan pays the administrative cost regardless of outcome.

What are my chances? A meaningful share of external reviews overturn the plan's denial, especially for medical-necessity disputes with solid doctor documentation. It's the fairest forum in the system — use it.

Put It to Work

If your internal appeal fails, don't deliberate — calendar the external review deadline immediately (typically four months) and file early. Gather the same packet that won or lost internally, plus anything new. Many states let you file through the department of insurance website in under 30 minutes. The reviewers are clinicians, not bureaucrats: write for a medical audience, lead with the clinical facts, and let your doctor's letter do the persuading.

How I Explain It to Clients

"This is the one the insurance company doesn't want you to know about," I say only half-jokingly. "Independent doctors, not company employees, decide — and their decision is binding." I walk clients through filing because the process favors the prepared: complete records, a strong doctor's letter, and filing well before the four-month deadline.
