What Is Durable Medical Equipment (DME)?
The one-minute explanation of durable medical equipment (dme) — no jargon.
Durable Medical Equipment (DME): Reusable medical equipment your doctor prescribes — wheelchairs, CPAP machines, glucose monitors, walkers — covered as an essential benefit with plan rules.
In Plain English
Durable medical equipment is reusable medical gear prescribed for a medical condition: wheelchairs, CPAP machines, hospital beds, walkers, blood glucose monitors, nebulizers. It's an essential health benefit, so Marketplace plans cover it — but with the plan's rules: in-network suppliers, prior authorization for expensive items, and sometimes rent-vs-buy decisions made by the plan.
A Real Example
You're diagnosed with sleep apnea and prescribed a CPAP ($900 device). Your plan requires prior authorization and an in-network DME supplier. Approved: you pay your DME coinsurance (say 20%, or $180), the plan pays the rest, and the supplier provides the machine plus supplies. Buy the same CPAP online out-of-network without authorization: $900 out of your pocket, counting toward nothing.
Why It Matters When You Shop
DME rules are a sleeper issue for anyone with chronic conditions — diabetes, sleep apnea, mobility limitations. When I review plans for DME-dependent clients, I check the DME coinsurance, the supplier network, and prior-auth thresholds, because a "cheap" plan with 50% DME coinsurance isn't cheap for them.
Watch Out For
Always use the plan's contracted DME supplier — retail medical-supply stores and Amazon may be out-of-network. And for big-ticket items, get the prior authorization before the supplier orders; retroactive approvals are painful.

Common Questions
Rent or buy — who decides? Usually the plan, based on cost and expected duration of need. Short-term needs (post-surgery walker) are often rentals; long-term (wheelchair) are purchases. The supplier handles the paperwork with the plan.
Are supplies covered too? Often yes — CPAP masks and filters, glucose test strips, wheelchair cushions. But supplies have their own rules and sometimes separate suppliers. Ask what's bundled with the device.
What if my DME claim is denied? Appeal it like any other denial — DME denials frequently involve medical-necessity documentation gaps that your doctor can fix with a better letter. External review applies here too.
Put It to Work
For any DME over a few hundred dollars: 1) confirm the supplier is in-network, 2) get prior authorization in writing before ordering, 3) ask whether the plan rents or buys (and what happens if your needs change), 4) keep the prescription and authorization with your records. DME done right is smooth; DME done out-of-network is one of the most expensive mistakes in the system.
How I Explain It to Clients
"Never buy the CPAP online first," I warn new sleep-apnea patients. "Use the plan's contracted supplier with prior authorization, or that $900 comes entirely out of your pocket." DME has the biggest gap I know between 'done right' (small coinsurance) and 'done wrong' (full price, counting toward nothing). One phone call before ordering decides which.
