Health Insurance for the Self-Employed: The 1099 Playbook
No employer plan, lumpy income, no safety net — here's how self-employed workers actually win at health insurance.
No Employer Plan? The Marketplace Was Built for You
If you're self-employed, you don't have an HR department shopping for you — which means you also don't have an employer picking a one-size-fits-all plan. The Marketplace lets you choose, and the tax credit math often favors 1099 workers more than they expect.
The 1099 Income Problem (and the Fix)
Your subsidy is built on estimated yearly MAGI — Modified Adjusted Gross Income. W-2 workers know their salary; your income is lumpy. The fix:
- Estimate from last year, adjusted. Last year's tax return is your starting point — then adjust for what's different this year.
- Count everything. All 1099s, cash and tip income, profit from the business. Underestimating feels good in January and hurts at tax time — especially now that 2027 repayment caps are gone.
- Update when it changes. Big new contract? Slow quarter? Update the application. Your credit adjusts with you.
- Don't forget the self-employed health insurance deduction. Ask your tax pro — premiums for self-employed coverage may be deductible, which changes the real cost picture.
The Stack: Health Plan + Supplemental
Self-employed means no workers' comp and no paid sick days. Most of my 1099 clients pair their Marketplace plan with AccidentWise (tiers up to $20,000) and HospitalWise — both Golden Rule plans with no medical underwriting, so they attach to almost anyone. The health plan handles the doctors; the stack handles the income interruption.
